Vacancy | Primary Care Mental Health Practitioner | Band 6 | Closes 19 February

A new and exciting role for an experienced Mental Health Practitioner to work as part of Central Liverpool Primary Care Network supporting a population of around 100,000 patients across nine practices. The role will provide support for people with significant mental illness to meet their needs, closer to their communities and homes, and is likely to be partly first contact (patients booked directly by Reception staff) and partly referrals from GPs. It will involve liaison with, but not limited to, practice clinicians, secondary care, Social Services, and the voluntary sector.
