LGBTQIA+ Social Prescribing Service

A specially developed service in a welcoming, friendly, safe environment, where you can get the support you need!

• We can connect you to local LGBT+ Services.
• Support for questioning identity or sexual orientation.
• Support with social transitioning.
• Signposting to hate crime organisations.

How do I get an appointment?

• You can book by sending an email to CLPCN.Admin@livgp.nhs.uk or you can ask your health professional to refer you for LGBT+ Social Prescribing.

Am I eligible?

• This FREE service is available to adults aged 18+ who identify as or are questioning being LGBT+ and their support networks/families.
• This service is only open to people who are patients at Central Liverpool Primary Care Network.

• Once we have received your referral, our team will contact you to arrange an appointment.
